NEW PATIENT INSTRUCTIONS

Are you interested in becoming a patient at Fora?

call 512-956-5006 or follow the steps below. We look forward to having you join the Fora family.

Please note that due to the high volume of requests, we only see patients who are actively trying to conceive, have infertility, or desire fertility preservation with IVF or egg freezing. Patients who do not live in Texas will only be seen if they are prepared to travel to Austin, Texas for fertility treatment.

You will need to register as a patient, complete your medical/fertility history, fill out consent forms, and request medical records from prior doctors.

STEP 1: COMPLETE THE NEW PATIENT REQUEST FORM

Your first step is to complete the form below. Please complete each field. After submitting this form, you will receive an email with a link to create a portal account. The email you select upon initial registration is where all patient communication will go, including the portal registration.

NOTE: IF YOU HAVE A PARTNER, WE NEED ALL OF THEIR INFORMATION. YOUR PARTNER WILL NEED THEIR OWN PORTAL ACCOUNT.

Once you submit the form, you will receive an email with further instructions. All patient communication at Fora is through the patient portal, if you have any difficulty in setting up your account please call 512-956-5006

STEP 2: CREATE A PORTAL ACCOUNT

After your new patient request form has been submitted, you will then receive an email to complete portal registration. The patient portal is essential for communication at Fora so please make sure you are using an email which you regularly check.

NOTE: IF YOU HAVE A PARTNER, YOUR PARTNER WILL ALSO RECEIVE AN EMAIL TO CREATE A PORTAL ACCOUNT. YOUR PARTNER NEEDS THEIR OWN PORTAL ACCOUNT.

Once you create a portal account, you will then be able to proceed to step 3. All patient communication at Fora is through the patient portal, if you have any difficulty in setting up your account please call 512-956-5006

STEP 3: COMPLETE PATIENT HISTORY

At Fora Fertility we believe in being prepared - this means we want to know you, your prior history, and your goals. After creating a portal account you will be able to complete the history sections on the patient portal including medical history, fertility testing and treatment, etc. Please complete the form in its entirety.

If you have a partner, they also need to complete all medical history through their portal account.

Your patient portal medical history section must be completed before being seen. Please fill out all sections to the best of your ability including uploading insurance cards and drivers license. You will then need to proceed to step 4: consents.

STEP 4: CONSENTS & MEDICAL RECORD RELEASE

To give you the best possible visit, Fora reviews all medical records available prior to your consultation. You will be sent a medical record release for any prior medical care that you have received. Once you have completed the patient history section, you can expedite your can by sending a portal message to “new patient scheduling” requesting consent forms. You will need to complete a request for any fertility clinic you have been seen at, your OBGYN, and any other medical specialists (if you have any medical problems managed be a specialist, complete a form for that provider).

Please note - if you have a partner, they WILL ALSO have consent forms TO COMPLETE. All FORA consents forms are required.

STEP 5: YOUR APPOINTMENT

We currently use telemedicine for all consultations, follow up visits, and classes.  Any visit which does not involve an ultrasound, blood draw, or other procedure is virtual at this time. Please make sure you have a strong internet connection and you are using a computer (not cell phone) for your visit.

IF YOU HAVE A PARTNER: PLEASE MAKE SURE THEY HAVE COMPLETED THEIR PORTION OF THE PORTAL AND YOU BOTH HAVE SIGNED CONSENTS PRIOR TO YOUR APPOINTMENT.

All patient consultations are all virtual.

THE PATIENT PORTAL

We are an electronic clinic and use the nABLE portal for all patient communication, release of lab results, cycle monitoring, etc. You will need to create an account and complete all paperwork. This is how we communicate so we are all on the same page and communication can be tracked.

We place patient handouts and all in-cycle instructions and release all labs to the portal, please make sure you are able to log into the portal and know where to find any information.

*all patients who previously had access to the athena health portal received an email with log in instructions for your nABLE portal account. if you are having difficulty setting up your patient portal account please call 512-956-5006.

    We need the following:

    Your medical history complete on the portal.

    Driver’s License

    Insurance Card

    Partner medical history, driver’s license, insurance card (if applicable)

    Consent forms (in your portal ready to sign).

    Release of medical information for any prior evaluation and treatment.

    *PRIOR IVF CYCLE RECORDS NEED TO BE RECEIVED BEFORE YOUR VISIT

    All new patient consultations are virtual and last 60 minutes.

    We currently use telemedicine for all consultations. Please make sure you have a strong internet connection, and you are using a computer (not cell phone) for your visit.

    If you have any difficulty getting onto your visit, please call the office for assistance (512) 956 5006. You can find the telemedicine link by going to forafertility.com and clicking on the “telemedicine” tab – you will then select your doctor.

  • Our standard business hours are Monday-Friday 7am-4pm. We are open on the weekends for patient procedures.

  • We believe that your time is valuable and we strive for you to have an efficient and comprehensive fertility experience. Monitoring appointments (appointments when patients are in a treatment cycle) or next steps appointments (completion of fertility testing) are both designed to move quickly so that patients can get on with their days. You should expect to be in the office for approximately 20 minutes or less with the goal of performing an ultrasound and/or bloodwork when you are in cycle for treatment.

  • You can schedule an appointment with either Fora Fertility physician who will carefully review your medical history, prior treatments, and work to craft a personalized treatment plan for you. Fora Fertility is a group practice, which means that Dr. Crawford and Dr. Skillern cover the clinical portion of “in cycle” appointments roughly equally. This means that you will see both doctors for your monitoring visits during your treatment cycle. This also means that the doctor performing your procedure (e.g., egg retrieval, insemination) will vary depending on the covering doctor the day your procedure falls. Our nurse manager, Melissa, also assists with some ultrasounds and procedures like inseminations.

  • We use the nABLE patient portal for all communication. This is the preferred means of communication so we are all on the same page and communication can be tracked. We place all in-cycle instructions and release all labs to the portal, please make sure you can log into the portal and know where to find all information sent via your clinical and administrative team.
